Form	You need this when:
Registration Form (players/coaches)	All registrations must now be completed online through your Member account. Contact your coach or team manager for your team's specific link or invite code to get registered.
Player Transfer/Removal	For player transfers and player removals. For Provincial & International Transfers, please refer to the Alberta Soccer Player Releases page for the process and required forms.
Minor Restricted Movement	A minor registered to CMSA will be playing up with your team NEW! (2023): only one copy of the minor restricted movement form needs to be filled out and submitted. The original copy will be given to the referee and the referee will submit it to the CWSA. Teams are encouraged to take a photograph of the form after the referee has signed it for their records.
Provincial Competition Opt-out	Teams in provincial contending divisions who wish to opt-out of provincials must submit this form to office@mycwsa.ca before the season's opt-out deadline.
Team Ownership Transfer	Your Team Owner wishes to transfer ownership responsibilities and privileges to another individual.
Team Name Change	Your team wishes to change their official team name.
Intent to Challenge	A team wishes to challenge for divisional promotion in the next season.
